Student success is shaped by more than what happens during the school day. When families face challenges with food, housing, transportation, health care or other basic needs, those barriers can affect attendance, engagement, behavior and overall well-being.
Social Care Networks, known as SCNs, are a New York State Department of Health initiative designed to help eligible students and families connect with non-academic supports that can improve health, stability and school success. Through regional networks across the state, SCNs screen individuals and families for health-related social needs and help connect them with available services.
